Please read this information in its entirety and follow the requirements outlined for outdoor visitation at Serenity House Assisted Living.
SCHEDULING
- Visitors must schedule their visit AT LEAST ONE DAY IN ADVANCE of the visit by going to the following website: https://serenityhouseassistedliving.simplybook.me. You must use this platform to schedule your appointment. Please note that this is on a first come, first serve basis. The main office staff and facility staff will be notified once you have booked your appointment.
- Visits during mealtimes and during medication administration periods and between 7:30am and 10:00am are not permitted as staff will not be readily available to facilitate the visit. Please arrive at your designated time; otherwise the visit may need to be rescheduled.
- If you need to cancel your visit, please click the link in the confirmation email that you receive so that we are notified of your cancellation.
OUTDOOR VISITATION PROCEDURES
- Self-Screening
- On the day of your outdoor visit, you must self-screen for symptoms by answering the following questions:
- Do you or have you in the last 14 days had any of the following symptoms?
- Cough, sneezing, shortness of breath, sore throat, or any of the other COVID-19 symptoms such as chills, muscle pains, headache, new loss of taste or smell, fatigue, gastrointestinal illness (nausea, vomiting, or diarrhea)
- Are you currently taking any fever-reducing medications?
- Have you had exposure to or reside with someone with documented or suspected COVID-19 in the last 14 days?
- Have you had a fever of 99° or higher in the last 14 days?
- If you answered “YES” to ANY of these questions, YOUARE NOT PERMITTED TO VISIT.
- If you answered “NO” to ALL of these questions and you do not have a fever (99° or higher) at the time of your visit, you may attend your scheduled visit.
- Do you or have you in the last 14 days had any of the following symptoms?
- On the day of your outdoor visit, you must self-screen for symptoms by answering the following questions:
- Mask Use – You must wear a mask or cloth face covering over your nose and mouth for the duration of the visit.
- Entry Procedure – Please ring the doorbell upon arriving at the facility. The staff member will meet you outside to perform an additional screening with a temperature check and symptoms questionnaire. If you pass the on-site screening, the staff member will direct you to the designated outdoor visitation area. Please proceed to the patio visiting area walking outside of the facility.
- Social Distancing – You must maintain a social distance of 6 ft. at all times during your visit. Please do not make physical contact with the resident. Please use the designated call button outside if you need assistance from a care staff member as well as to notify the care staff that you will be leaving.
- At no time are you permitted to leave the designated visitation area on the patio to interact with residents or staff members at other locations on the patio.
- All visitors are permitted to stay for a maximum of 45-minutes. Please arrive at your scheduled visit time. This is extremely important for scheduling purposes and resident safety. Please be respectful of other family members and essential healthcare personnel who also have scheduled visits throughout the day
- If you bring food or beverages to your family member, we will give it to them at a time after your visit. Unfortunately, at this time you cannot share these items with your family member as it will require you and the resident to take off your masks.
- Any visiting child that is not being held (or is in a stroller/baby carrier) is required to follow the same precautions such as: wearing a mask, not touching the resident, and maintaining at least a 6 ft. physical distance.
- Due to governmental public health orders and load capacity on the patio, only two visitors are permitted to be visiting their family member at a time.
